Email Subscriptions

Email and text message subscription services are offered by Health Smartz Online as a convenience to users who choose to receive information via these channels. All information published via email subscriptions and text messaging services is available on the Health Smartz Online website without the requirement to provide email address or phone number.

In order to manage Health Smartz Online email subscription lists, we retain the names and email addresses of our email subscribers, as well as other information they may have shared with us (for example, addresses and phone numbers), logs of emails we send, and automatically generated email data used to improve our email communications. Survey Tools

Invitations to Health Smartz Online surveys are sent via a pop up or external links to a random sampling of visitors. Your acceptance of a survey is completely optional. If you decline any survey, you will still have the same access to the information and resources provided by Health Smartz Online, as those who opt to take a survey.

Health Smartz Online uses online survey providers to collect opinions and feedback from visitors. These survey service providers obtain feedback and data on visitor’s satisfaction on behalf of Health Smartz Online. Surveys conducted on behalf of the Health Smartz Online do not collect any personally identifiable information (PII), and the aggregate results are only available to Health Smartz Online staff who require this information to perform their duties.

Mobile surveys (that is, those accessed via your mobile device) may request an email address and a phone number from you in order to send an external link to the survey. Health Smartz Online does not receive your personal information, nor will the Health Smartz Online have access to your information.
